Anesthesia & Surgical Release Form

  • As Owner (or agents for the owner), I hereby authorize the veterinarian(s) employed by Meadowrun Affordable Pet Care perform the necessary diagnostic, therapeutic, surgical procedures deemed advisable or necessary for my pet.

    I certify that I own the above-described animal and I do hereby consent and authorize Meadowrun Affordable Pet Care and their staff to hospitalize my pet, administer any vaccinations, medications, tests, surgical procedures, anesthetics or treatments that the doctors deem necessary for the health, safety or well-being of the above animal while it is under their care and supervision.

    I further realize that I am responsible for payment in full for the above-mentioned procedure(s) and required treatment at the time the animal is discharged. If I neglect to pick up the animal that was scheduled and ready for release within 10 days of written notice (mailed to the above current address), Meadowrun Affordable Pet Care will assume that the pet is abandoned and is then authorized to release my pet to other facilities, pursuant to FL Statute #705.19.

    The abandonment of the pet does not release me of my financial obligation.

    I further agree that in the case of non-payment, a finance charge of 2% per month or 24% per year will be charged and that any collection fees or attorney fees will be my financial responsibility.

    ***ALL ANIMALS ENTERING THIS HOSPITAL MUST BE CURRENT ON THE REQUIRED VACCINATIONS AND FREE OF INTERNAL AND EXTERNAL PARASITES OR THEY WILL BE TREATED AT THE OWNER’S EXPENSE.***

    From organ dysfunction and dehydration to low glucose reserves, many conditions may predispose your pet to anesthetic complications. Testing will alert the doctor of the potential for any problems. Therefore we perform the following:

    I. A surgical catheter and fluid therapy for all our surgical patients. Placement of a catheter provides immediate access to the circulatory system in the event of complications or adverse reactions during anesthesia. Fluid therapy maintains blood pressure, prevents dehydration, provides support to the liver and kidneys and enhances recovery from anesthesia.

    II. Pets 6 years of age and less: Pre-operative CBC and select function blood panel

    III. Pets 7 years old and up: Pre-operative CBC and full function blood panel

    IV. As with people, animals experience pain. To ease discomfort, we require that all surgical patients receive pre-surgical pain medications and pain management medications to take home.
